What to Bring to Your Appointment
To help us verify your benefits, please bring the following to your visit.

This allows our team to add your insurance information to your profile and confirm eligibility.
Important Things to Know About Insurance
Insurance plans differ widely in what they cover and how they apply benefits. Chiropractic coverage may include limitations such as visit caps, deductibles, copays, or coinsurance.
Our team will do our best to verify your benefits, but final responsibility for payment remains with the patient based on your individual insurance plan.
Massage therapy and other services may not be covered by insurance and are typically self pay unless otherwise specified.
Self Pay and Flexible Options
If your insurance is not listed or if you prefer not to use insurance, we offer self pay options and flexible care plans. Our team can discuss pricing and payment options with you so you can choose what works best for your situation.
